Advanced Orthokeratology

PauneVisión proposes the Double Reservoir Lens (DRL) to overcome the problems and disadvantages associated with currently-used lenses. A predictable and logical system, the DRL simplifies the fitting of the lens, improving centering and providing greater control in the fluorescein pattern. DRL lenses are a patented design.

One of the great difficulties of currently-used ortho-k lenses is the great skill needed by the doctor. This is because the lenses must be fitted with a sagittal error lower than 5 microns, causing runout problems when they are loose and corneal distortion when they are adjusted. Both result in poor visual quality. Ortho-k is especially difficult when the cornea has astigmatism, because the corneal sagittal height is different in the two axes, producing a poor fit and visual distortion.

The current technique, however, is limited regarding the amount of myopia reduction, the speed of correction and the final optic zone diameter.

DRL lenses allow direct control of the fluorescein pattern, which no other ortho-k system allows. The treatment speed is accelerated by the second tear reservoir. And not only does it treat nearsightedness, but it is the only comprehensive system in Ortho-k that also treats astigmatism, farsightedness and overcorrections or undercorrections as a result of LASIK or Refractive Surgery .

Innovative Design

The fundamental characteristic of DRL is their peripheral curves which are designed to generate two rings of tears, contained between the inner surface of the lens and the corneal surface.

This provides a greater fitting tolerance of the sagittal height of the lenses. Corneal epithelium modifies easily below the surface of the lens.

The lenses provide a high control of the fluorescein pattern; The comparison of the two rings or touch points shows the exactness of the fit on the fluorescein pattern, (steep, optimal or flat), which allows noticeable changes of 10 microns (0.05 mm radius).

Its unique design and geometry allows optimal centering. DRL lenses also give a good treatment zone diameter.

Double Reservoir Lens vs. Alignment Curves

A previous comparative study determined that on average DRL lenses give greater reduction than Alignment Curves (Presented in 2004 at the Global Orthokeratology Symposium and published in the Gaceta Optica nº 392, 2004).

These lenses also achieve a greater myopia reduction than current lenses, due to the suction forces generated in the two reservoirs.

Software and IT support

DRL PauneLens lens is fitted by calculating the corneal sagittal height, using central apical radius and eccentricity in a specific software or directly into the software available on Keraton and Medmon topographers. With this data it is decided which of the 24 trial set lenses is the optimal. Then we check with the fluorescein pattern.

It is posible also to fit directly over mean K radius; In a cornea with a mean K equal to 7.80 mm. the first choice lens from the trial set should be K 7.80 M 2.

All the lenses are named whit a "K" number and a "M" number. This nomenclature means; Lens designed to fit over a normal cornea (Average eccentricity of 0.48) with an a mean Keratometric radius of "firts number" and a myopia of "second number". On the above example this correspond to K 7.80 M 2.

We use in the manufacturing process Nanotechnology lathes capable of cutting extremely accurate surfaces avoiding the need for polishing and laser interferometry controls that ensure the reproducibility and accuracy. The lenses are graved for easy identification. A serial correlativie number is over each lens.

Parameter Flexibility

Using the concept of equivalent lens (all the lenses with the same K radius have the same sagittal height) the fitter can change easily and securelly between the various possible choice parameters.

11.20, 10.80 (Standard) and 10.40. This lenses have a 6.60 mm optic zone

11.00, 10.60 and 10.20 mm have a 6 mm Optic Zone.
